Paperwork for Psychotherapy Clients



If you will be receiving teletherapy services, Beth will provide you the HIPAA- compliant website link to complete the paperwork during the phone consultation.



All required paperwork is accessible through the link below. Before coming to the office, please print and read through the paperwork, and initial and sign where indicated. Please bring the completed paperwork with you when you arrive to your session, because our session cannot begin until the paperwork is completed. Please bring a paper copy – do NOT email or text the paperwork, because these are not secure forms of communication.

If you and your partner/spouse are coming together, you will each need to print, sign, and bring your own copies of the paperwork.

Late Cancellations: Beth Christopherson’s business is her livelihood. Beth Christopherson, LCSW, PLLC provides one free late ( i.e. within 24 hours) cancellation but must charge the full price of the session for any additional late cancellations, regardless of the reason for cancellation. Beth understands that clients always have good reasons for when they have to cancel last-minute. However, Beth does not have the ability to offer more than one free last-minute session cancellation. If an initial/assessment session is cancelled within 24 hours, a charge of 160 will be made as Beth reserves 2 hours for new-client appointments. Thank you for your understanding.
